Name Change
UT San Antonio recognizes that many of its students use a name other than their legal name. As long as the use of a preferred name is not for the purpose of misrepresentation, the university acknowledges that a preferred name should be used whenever possible in the course of university business and education.
Request to Change your Legal Name
Official changes to the first, middle, and/or last name will require submission of a Biographical Data Change Request Form form and at least two forms of supporting documentation indicating the new legal name (two primary identifications or one primary + one secondary).
Requests submitted without necessary documentation will not be processed.
