UTSA Name Change
UTSA recognizes that many of its students use a name other than their legal name. As long as the use of a preferred name is not for the purpose of misrepresentation, the university acknowledges that a preferred name should be used whenever possible in the course of university business and education.
Request to Change your Legal Name
Official changes to the first, middle, and/or last name will require submission of a Name and ID Number Change form and at least two forms of supporting documentation indicating the new legal name (two primary identifications or one primary + one secondary).
Requests submitted without necessary documentation will not be processed.
Primary Identification (Photo ID) | Secondary Identification |
---|---|
State Issued ID Card Driver’s License Passport Military ID Certificate of Naturalization |
Birth certificate Marriage License Divorce Decree Court Order Social Security Card |
Students who are current or former employees of UTSA must also visit Human Resources to change their name.
Request to Submit a Preferred Name
The University of Texas at San Antonio recognizes that some students may choose to identify themselves within the UTSA community using a chosen or a preferred name that differs from their legal or official name. Students can submit a preferred name online through the Personal Information tab within their myUTSA Account.
Submitted preferred names will be reviewed by the Office of the Registrar. Names containing obscene language or used for the purpose of fraud/misconduct will be removed and reverted to the legal name.
Once your preferred name request has been approved by UTSA, it will appear in the following locations:
- myUTSA Account
- Blackboard
- Class Roster
- Class Photo
- Mid-term grade report
- Final grade report
- UTSA Mobile App
Your preferred name will not appear on your official transcript or your diploma.
Please note that third-party sources may attempt to contact students on behalf of UTSA by utilizing the legal name. Based on FERPA policies, student can restrict directory information.
