Guidelines for Submitting Forms & Paperwork
Forms that are interactive should be filled out
online and printed prior to submission.
Unless otherwise indicated on the form itself, submit the completed form through our online Document Uploader.
All forms (and supplemental documentation) must have the student’s name and myUTSA ID (abc123 format) on the top of each page.
You may review what forms are necessary to complete your financial aid file on your ASAP account.
Guidelines for Submitting Citizenship Documents
If you need to submit citizenship documents (e.g. passport, birth certificate, certificate of birth abroad, permanent residency card, certificate of naturalization, etc) please realize the originals must be brought in person and we will make necessary copies and return the original to you. Any copies of citizenship documents submitted by fax, mail or drop box will not be processed.
Important Note about Forms
Forms that are interactive should be filled out online, printed, and physically signed prior to submission.
Do not sign forms electronically. Any forms submitted without the proper signatures will not be processed and will need to be resubmitted.
NO DOCUMENTS will be accepted via email.
