Social Security Number Issue

The National Student Loan Data System (NSLDS) found your reported Social Security Number (SSN) on their database, but your name and date of birth did not match. Therefore, this Student Aid Report (SAR) does not contain the financial aid history that is associated with your reported SSN. Please note that the Student Aid Report (SAR) will be called the FAFSA Submission Summary starting with the 2024-2025 school year.

1. Please provide ONE of the following to the One Stop Enrollment Center:

• A copy of your SSN card
• A copy of your driver’s license/identification card
• A copy of your Certificate of Naturalization
• A copy of your court order petition for name change

2. Submit your chosen official document from above via our Financial Aid Document Uploader.

1. Scroll past the instructions and click the green button “Upload a new document”
2. Fill in these values:
   • Department = “Financial Aid & Scholarships”
   • Term = the relevant aid year
   • Category = “Financial Aid Hold”
   • Category 2 = “SSN/Name Change Hold”
   • Documents = “SSN/Name Change Hold Document”
   • Choose File = select the file on your computer/device. Must be less than 10MB size as a JPG, PDF, or TIF
3. Press the green “upload” button. You’ll see confirmation of the document uploaded back on the home screen under “Upload History”.
