As a result of your SAP appeal, you will be placed on an academic plan.  The SAP Plan Agreement is a form that details the length and requirements needed to be completed for your academic plan.  All requirements listed on the agreement must be met at the end of each term.  The requirements will be checked when grades are made official and academic standing is updated for the term.  Failure to meet the requirements will result in termination of financial aid for the following and future terms.

Please read, print, sign and submit the SAP Plan Agreement to One Stop Enrollment Center.  Your financial aid will not be awarded until the form is received and processed.  It is advisable to make payment arrangements with fiscal services to ensure that classes will not be dropped for nonpayment by the payment deadline listed for the term.

You can submit your agreement one of the following ways:

For data security purposes, documents submitted by email or fax cannot be accepted.
